Email records to vision@periopeak.com

A full mouth series of x-rays (up to18 individual digital x-rays) and photos if you have them.
A comprehensive periodontal charting to included pocket depth and mobility.
Contact information for the patient to include email and phone number.

We only accept digital records.

Step 2

Consultation

Once the patient's records are received we will make contact to schedule a phone consultation within 24-48 hours.

We will gladly explain the process and answer any questions your patient may have.

Step 3

Make Appointment

Once we've provided a phone consultation your patient can schedule an appointment by email or phone.
